Myoma submucous: the main symptoms and description. Treatment Scheme and Consequences
Myoma of the uterus - a fairly common and dangerous pathology in women, characterized by the appearance of a benign neoplasm. There are several different types of myomatous nodes, one of which is submucous myoma, it can actively develop and expand.
This disease is a benign tumor that protrudes beyond the uterus, formed from its muscles. There may be multiple fibroids, which consist of several nodes at once, and they are located immediately in several zones.
Features of submucous uterine myoma
Submucous myoma - a benign neoplasm, which is formed in the submucosal layer of the uterus. The peculiarity of this disease is that fibrous nodes expand not into the cavity of the uterus, but towards the abdominal cavity.
If the submucosal nodes are large, they squeeze the ureters and intestines, which is manifested in problems with urination and defecation. In addition, such a neoplasm can lead to a dead pregnancy or provoke a miscarriage.
The causes of the formation of submucous myoma
- A change in the hormonal background;
- Frequent stress;
- physical exercise;
- obesity;
- Reception of oral contraceptives;
- Absence of pregnancy and childbirth till 30 years;
- Hereditary factor;
- Frequent abortions;
- Inflammatory diseases of the genital area.
The main reason for the formation of myomatous nodes can only be determined by a doctor. It is very important to determine what triggered the pathology, since this will prevent the occurrence of it in the future.
The main signs of the disease
- Prolonged and profuse menstruation;
- Painful sensations in the lower abdomen;
- Presence of blood clots in menstruation;
- Severe uterine bleeding;
- Bloating;
- Increase in temperature;
- Weakness and malaise;
- infertility.
In some cases, submucous myoma does not manifest itself at all, especially in the initial stages. With the development of pathology, the symptoms begin to grow more and more. It is very important to conduct diagnostics in time to determine the presence of pathology, as this facilitates rapid treatment and recovery.
Diagnosis of the disease
Myoma with submucous growth is diagnosed by gynecological examination using the method of palpation. In this case, the doctor can detect a myomatous node of a dense consistency.
To make a more accurate diagnosis, an X-ray is performed, as the picture shows an enlargement or curvature of the tumor's shadow. Submucosal nodes in the image are defined as clear defects with clearly visible contours.
One of the most informative diagnostic methods that accurately helps determine whether there is an interstitial-submucous myoma is echography. The informativeness of this method is quite high, since newer and more modern tools and techniques have appeared. Ultrasonic scanning makes it possible to detect myomatous nodes, even if they have a small diameter.
To clarify the diagnosis, an ultrasound tomogram is performed, since it is possible to obtain a three-dimensional ultrasound image. This is a fairly informative method, since it allows us to fully evaluate the ongoing pathological process and to clarify whether there is a concomitant pathological process.
Treatment of fibroids
The submucous myoma was previously treated only by the operative method. When diagnosing the myomatous node, only the removal of the uterus was performed before, after which the woman could no longer give birth to the baby.
The disease is very poorly amenable to therapy with medications, but hormone-containing drugs can somewhat stop its further development. The use of hormonal drugs can provide in the female body such a hormonal background, which will correspond to the period of menopause.
If the patient has submucous myoma, treatment with medicinal hormonal preparations can bring the desired effect if the patient is more than 40 years old and is close to the menopause period. With successful therapy, there is a high probability that the myoma will begin to decrease in size and eventually disappear completely.
Many patients prefer to carry out treatment with the help of folk techniques, but initially it is necessary to consult with the doctor, since incorrectly selected therapy can significantly worsen the patient's condition and provoke complications.
Medication
If a patient with fibroids, a submucosal node larger than 5 centimeters, does not undergo surgery, since a variety of complications can occur.
In such a case, medicamentous treatment is performed and the patient is prescribed drugs, for example, "Zoladex" or "Decapeptil-Depot", which allow creating an optimal hormonal background. In this case, the level of estrogens is significantly reduced, which contributes to the reduction of fibroid in size.
In addition, the patient may be prescribed special drugs that are administered intramuscularly or intravenously. In some cases, when the nodes are small, hormone-surgical treatment is indicated.
Women of reproductive age with severe disease are prescribed the drug "Esmia." However, it has some contraindications, namely:
- Pregnancy and the period of breastfeeding;
- asthma;
- oncological diseases;
- Vaginal bleeding.
This drug is not allowed to be used for a long time, since it can provoke serious complications.
Operative intervention
If the submucous uterine fibroids are quite large, the operation is mandatory, since it can be a serious threat to life. Myoma can be removed by laparoscopy or hysteroscopy. Laparoscopy is carried out through the front wall of the abdominal cavity, and hysteroscopy - through the uterine cavity.
If these methods do not bring the desired effect or if there are indications, then a complete removal of the uterus. Myoma can have very serious consequences, therefore it is necessary to conduct a patient's examination and treatment as soon as possible.
Features of hysteroresectoscopy
The procedure for hysteroresectoscopy is considered to be one of the most gentle methods of surgical intervention and is performed to preserve the reproductive function, as this is important for the subsequent planning of the birth of the child.
Such surgical intervention is carried out through the vagina and cervical canal. For the introduction of instruments the cervical canal widens slightly. The duration of the surgery is no more than sixty minutes. If in this way the submucous myoma is removed, the responses of the patients are only the most positive, which is very important for patients planning a pregnancy.
Carrying out a mechanical myomectomy
Submucosal nodes of fairly large size are removed mechanically. Initially, the node is fixed using special tools, and then unscrewing under the control of the hysteroscope.
Capsules can be dissected using a special resector and removal of the node. Removal of submucous myoma by mechanical myomectomy has its definite advantages, namely:
- Short duration;
- No need to use additional equipment;
- There are no complications.
After the operative intervention, the recovery period does not take much time. In addition, there is no damage to large organs, as well as the overload of blood vessels.
Features of electrosurgical myomectomy
Electrosurgical myomectomy is performed if the node is in the 1 st or 2 nd stage. In addition, in this way, removal of nodes located in the corners of the uterus is performed, which can not be removed in any other way.
Operative intervention can be carried out in one or several stages, it all depends on the nature of the site. When the submucous uterine fibroids are at the advanced stage, the reviews of this method of treatment are quite good.
Other methods of surgical intervention
Other methods of surgical intervention can also be performed in different stages of myoma. If none of the sparing surgical methods yields the desired result or if there are certain indications, then the uterus is completely removed along with the cervix.
At full removal, vaginal, abdominal, and also laparoscopic access is used.
Submucous fibroids as a cause of infertility
- Submucous;
- Intermuscular;
- Subserous.
With submucous myoma the nodes can be either on a wide base or on a thin stem. This imposes a certain imprint on the overall clinical picture of the disease. This is a rather complex and dangerous pathology, which poses a serious threat to the health and life of the patient and leads to infertility.
The main causes of infertility
Submucosal nodes with uterine myoma lead to infertility for several reasons. To such reasons it is possible to carry:
- Mechanical factor, as there are problems with implantation of a fertilized egg;
- Hormonal disorders, which leads to problems with ovulation;
- Closure of the opening of the fallopian tubes, which significantly complicates the process of movement of the egg to its implantation site;
- There is a risk of developing an ectopic pregnancy;
- Increased endometrial activity;
- Immunity disorders, which negatively affects the implantation process.
In addition, this pathology can lead to miscarriages due to the compression of the uterus, internal organs, and certain hormones that interfere with the normal bearing of the child.
Thus, uterine myoma puts a serious imprint on the activity of the reproductive system. That is why it is necessary to start treatment of the disease as soon as possible.
