Corinfar. Instructions for use

"Corinfar" is a calcium antagonist, that is, it inhibits the penetration of Ca2 + ions into muscle cells (cardiomyocytes) of the heart and vessels from the intercellular space. Thus, there is a decrease in the concentration of calcium ions. As a result, peripheral, coronary arteries and arterioles (soft arteries) expand. Tablets, having a prolonged action, have a pronounced vasodilator effect.

The medicine "Corinfar" is produced in the form of round biconvex tablets, covered with a yellow film membrane. One tablet contains the active substance - nifedipine (synthesized artificially from dihydropyridine) - 10 mg. As auxiliary substances filling the tablet, lactose monohydrate, povidone K25, potato starch, microcrystalline cellulose, magnesium stearate are used. "Corinfar" is sold in a cardboard box with three blisters (10 tablets) or in bottles of dark glass of 50 and 100 tablets.

With such diseases as stress angina, hypertension (persistent high blood pressure), patients are prescribed Corinfar. The instruction for use is in Russian. It is known that the curative effect will be delayed (without reducing the absorption of the active substance - nifedipine), if during a meal take "Corinfar". Instructions for use recommend doing this (without chewing) after eating and drink plenty of liquid. The duration and dose (that is, the course of treatment) are selected individually and appointed by the doctor. Correspondingly sensitivity to the Corinthian and the severity of the disease are taken into account. For elderly patients and those who suffer from concomitant severe cerebrovascular disease, the dose is reduced.

Begin with 10 mg of nifedipine (or 1 tablet two or three times a day) to take Corinfar. Instructions for use allow a gradual increase in the dose to 2 tablets or 20 mg once or twice a day. The daily maximum dose should not be more than 4 tablets or 40 mg. In essential hypertension (hypertensive disease), the recommended daily average dose should be 1 tablet or 10 mg (it is considered for nifedipine) two to three times a day. In order to achieve the required clinical effect, the dose to the doctor's prescription can be increased to 40 mg (according to nifedipine) per day, which should be taken 2 times. Between the two methods of the Corinfar, with a two- or three-fold appointment, the time interval should be at least four hours.

In some diseases it is not recommended to prescribe Corinfar. Instructions for use indicate its contraindications for arterial hypotension, cardiogenic shock, collapse, chronic heart failure, with severe aortic stenosis, unstable angina, acute myocardial infarction (during the first four weeks), joint application with rifampicin. There are contraindications also during lactation, in the first trimester of pregnancy, with increased sensitivity to nifedipine or to the auxiliary components of the drug.

Special care is required with hypertrophic obstructive cardiomyopathy, mitral valve stenosis, sinus node weakness syndrome, severe tachycardia or bradycardia, hypovolemia, malignant arterial hypertension, myocardial infarction with left ventricular failure, severe cerebrovascular accident. Also, in renal and hepatic insufficiency, gastrointestinal obstruction, hemodialysis, during the II and III trimesters of pregnancy, at the age of 18, with simultaneous reception of beta-adrenoblockers, digoxin.

A drug with a pronounced hypotensive and antianginal effect with a prolonged effect due to a delayed action of the active substance is called "Corinfar retard." The instruction contains information for health workers and patients. The active substance of the preparation is easily absorbed into the digestive tract. The therapeutic effect develops no more than 15 minutes after taking the medication. Metabolism of the components of "Corinfar" occurs in the liver due to hydrolysis and oxidation. In the metabolism of nifedipine, inactive compounds are formed. "Corinfar" is excreted mainly with kidneys, as well as with feces.
