The drug "Torsid": instructions for use, description, composition and reviews. "Torsid": instructions for use (tablets)

Recently, modern doctors have increasingly begun to prescribe to their patients a drug such as "Torsid." Instruction for use includes only the basic data needed by a person for normal administration of the drug, but many people try to find out more information about this drug before purchasing or after purchase.

Pharmacodynamics

This drug is a saluretic. It is responsible for inhibition of the reverse renal reabsorption of sodium and chlorine ions located in the ascending part of the Henle loop, as indicated in the Torsid drug instruction manual.

Pharmacokinetics

In humans, after the intravenous administration of this agent, the diuretic effect quickly begins to manifest itself. In this case, the limiting effect of the drug develops throughout the first 3 hours and persists for up to 12 hours. Against the background of the use of this drug diuresis is directly dependent on the dose, which also indicates the drug "Torsid" instructions for use.

The degree of binding of torasemide to plasma proteins reaches 99%, which is a very good result, while the metabolites M1, M3 and M5 bind respectively to 86.95 and 97%. The maximum possible half-life time of torasemide, as well as its various metabolites in healthy subjects, is three to four hours, as determined during clinical trials.

The total clearance of this substance is approximately 40 ml / min, while the renal clearance is about 10 ml / min. Metabolite M5 does not produce any diuretic effect, while the combination of metabolites M3 and M1 accounts for approximately 10% of the total pharmacodynamic effect exerted on the human body.

Features of the drug in people with various diseases

If the patient has renal failure, this in no way affects the overall clearance and half-life of torasemide, while this period increases for metabolites M5 and M3. At the same time, the original pharmacodynamic characteristics are completely preserved, but the duration of the action of the drug kidney failure also has no effect, regardless of the severity of the disease.

In those patients who experience violations of the liver function or have heart failure, the periods of excretion of the metabolite M5 and torasemide slightly increase, as indicated in the Torsid drug instruction manual.

The amount of substance excreted in the urine is almost exactly the same as that observed in healthy volunteers, and therefore torasemide itself and its metabolites in no way cumulate.

Indications

In most cases, this drug is prescribed by doctors in the treatment of various effusions or swellings that have appeared in connection with heart failure. It is worth noting that the "Torsid" preparation instruction for use (tablets) indicates that it is prescribed when necessary for standard therapy.

Contraindications

There are several cases in which the use of this drug is not recommended or even prohibited. They are described in the instructions for use (tablets - the form in question) attached to the "Thorside" preparation, it is worth highlighting several basic ones:

  • The patient has arterial hypotension;
  • Excessive sensitivity to various auxiliary or active substances, as well as other drugs of sulfonylurea;
  • It is not recommended for use in patients with renal insufficiency with anuria;
  • With severe violations of the liver;
  • All kinds of violations in the system of water-electrolyte balance;
  • gout;
  • If the patient suffers from severe impairment in the urinary system;
  • All possible disorders of the conduction system of the heart of a high degree;
  • With pathological changes in the general picture of blood and acid-base state of a person;
  • Disorders in the functioning of the kidney caused by the defeat of various nephrotoxic substances.

Among other things, it is not recommended to use this tool for women who are in the period of pregnancy or breastfeeding.

How is it used?

If a liquid form is used, the remedy should be administered exclusively intravenously and slowly, as indicated by the Torsid preparation instructions for use. The price of this facility (from five hundred rubles) is quite affordable, which is why most people should not have any problems in order to properly observe the indicated indications for its use.

The initial single dose is about 10 mg per day, that is, 2 ml of this drug. If there is a slight clinical effect, then the dose can be doubled. If this also does not allow to achieve proper effectiveness, then in this case it is possible to increase the dose two times more, but the therapy should last for a maximum of three days.

Acute pulmonary edema

If this remedy cures acute pulmonary edema, then the starting dose is 20 mg, depending on the overall clinical picture, this dose can be administered every half hour. It is worth noting that with this treatment, the total dose should not be more than 100 milligrams of torsemide per day.

For subsequent treatment, it is recommended to switch to oral administration as soon as possible, since intravenously it is not necessary to use the remedy for more than a week, which also marks the instruction for use with the Torsid preparation. The price (reviews this confirm) of both tablets and liquid substance is available to many, so a full course of treatment rarely causes any difficulties. In any case, the maximum possible daily dose should not exceed 100 mg.

Liver failure

In the case of using the remedy for patients with hepatic insufficiency, it is necessary to apply it as carefully as possible, since in this case there is a risk of an increased concentration of torasemide in the blood plasma. This separately notes to the drug "Torsid" (10 mg) instructions for use (tablets or liquid form of the drug - it does not matter).

Elderly people

For treatment of this category of patients, there is no individual approach to the dose selection process. It is worth noting that no serious clinical studies have been conducted regarding the comparison of the treatment procedure in young and old people, and it is always better to consult a qualified physician beforehand.

The solution for injections should be injected slowly intravenously, and in this case it is by no means intraarterial, which separately emphasizes the instruction for use (ampoules) to the Torsid preparation.

Side effects

It is worth noting the fact that adverse reactions can affect the work of various organs, so each case will be considered separately:

  • The cardiovascular system. In this case, when taking the drug, various disturbances of the water-electrolyte balance can be present, and in some isolated cases metabolic alkalosis also increases. In addition, a certain category of patients has confusion, thrombosis, arterial hypotension, cerebral and cardiac ischemia, acute myocardial infarction, angina and syncope.
  • Nervous system. In rare cases, and especially at an early stage of treatment, headache, excessive fatigue, dizziness and muscle cramps can occur.
  • Digestive system. In rare cases, there may be abnormalities in the digestive system during the use of the drug "Torsid." Instructions for use notes the following side effects: nausea, lack of appetite, vomiting, diarrhea, stomach pain. An excessively high level of hepatic enzymes may also be present.
  • Urinary system. In patients experiencing various disorders in the urinary system, there is a fairly intense urination, which ultimately can lead to its delay, as well as too much stretching of the bladder. In rare cases, there may be an overestimated rate of creatinine and urea in the blood.
  • Metabolism. In rare cases, the patients recorded an increase in the total level of uric acid, lipids and glucose in the blood.
  • System of blood. In extremely rare cases, the total number of erythrocytes, white blood cells and platelets decreases.
  • The immune system. In rare cases, there is exanthema, photosensitivity, itching. To the preparation "Torsid" (nyxes), the instruction for use indicates that in some cases, after acute intravenous injection, acute reactions of hypersensitivity may develop, so that the patient should receive the fastest possible help. In certain cases, the occurrence of severe skin reactions.

If we talk about less significant effects, then rarely there is paresthesia and dry mouth. In addition, special attention must be paid to the fact that in extremely rare situations, people have a certain visual impairment, as well as the appearance of tinnitus or complete loss of hearing.

Overdose

The typical symptomatology in case of an overdose of the drug is currently unknown. The instruction indicates that in the case of an overdose there may be a sufficiently strong diuresis, as well as a risk of severe loss of electrolytes and water, drowsiness and an amnesic syndrome. Among other things, with an overdose , cardiovascular failure, arterial hypotension and all kinds of disorders in the digestive tract work.

How to be treated?

There was no specific antidote at the moment. In most cases, the symptoms resulting from intoxication completely disappear after the dosage is reduced or the drug is completely discontinued, and if adequate replenishment of the electrolytes and liquid is provided, which should be done under maximum control conditions. Torasemide is in no way removed from the blood by hemodialysis.

This is the basic information regarding the drug "Torsid" (instructions for use, price, reviews). Analogues of this drug ("Britomal", "Sutrilneo") in most cases are characterized by a similar action and side effects, but the doctors most often appointed this tool as one of the most effective in their group.
